Ordering a Computer

**This application is for individual purchases only. If you are purchasing from an Agency or with a Purchase Order, please follow this link.

Please fill out the following order form. Select the machine and other options that fit your needs. Please note, some options may not be in stock. After you submit your order, you will receive a call from a Customer Service agent to complete the order. To be eligible for a computer from us, we require a doctor's note or equivalent document that verifies your vision level. You can submit that document with this form, or send it to us when we contact you. The subtotal shown with this order is an estimate and we will confirm the amount during the phone call. Please do not submit any payment until that time.
